Complete your patient intake forms online — it takes about five minutes and saves time when you arrive. Bring a valid photo ID and your insurance card if you have one. If you're on any medications or have a relevant medical history, please disclose it on the intake so we can plan the safest possible care.
Plan for about an hour for a comprehensive new-patient exam. We'll do a full visual exam, take digital X-rays, evaluate your gum health, and walk you through what we see — including any treatment options, risks, and pricing — before any work is recommended. We never rush you, and we never push services you don't need.
We offer a complimentary Virtual Smile Consultation for cosmetic cases — send a few photos of your smile and Dr. Amjad will share an honest assessment and treatment options. For complex cases or second opinions, ask the front desk and we'll see what we can arrange.
It depends on what you need. Routine exam-and-cleaning visits with PPO insurance are typically covered at 100% — we file the claim for you. For uninsured patients, ask about GleamBar, our in-house dental membership ($99.99/mo bi-annually or $200/mo month-to-month) that includes unlimited exams, cleanings, X-rays, and 3D scans. For larger treatment plans, we offer 0% APR financing through CareCredit (up to 24 months) and HFD (up to 60 months).
A returning-patient cleaning and exam usually takes 45–60 minutes — we don't compress it into the 20-minute "production" model some offices use. The extra time means a more thorough cleaning, better detection of small issues before they become big ones, and a chance to actually talk through anything you're noticing about your teeth or gums.
We accept all PPO plans — including Delta Dental, Cigna, United Concordia, UnitedHealthcare, Guardian, GEHA, Sun Life, Ameritas, Principal, Lincoln Financial, Careington, Humana, and Mutual of Omaha. As long as it’s a PPO plan, most let you choose your provider. Even if your specific plan isn’t listed, as long as it’s a PPO plan we are still able to bill your plan out. Fill out your intake forms with your insurance card and we’ll verify benefits before your appointment — for free.
No, a COVID test is not required for routine dental care. We do ask all patients to complete a brief screening questionnaire before each visit, and we maintain enhanced infection-control protocols — medical-grade air purification, contact-free checkout, surface disinfection between every appointment, and aerosol-reduction equipment on every operatory.
