1.1 Financial policy

Thank you for selecting our office as your dental care provider. We are dedicated to delivering the highest quality dental care to support your long-term oral health. Payment is due at the time of service unless prior financial arrangements have been made.

We accept cash, personal checks, debit cards, HSA/FSA, VISA, American Express, MasterCard, and Discover. A 3% processing fee applies to credit card transactions; debit card payments are surcharge-free.

We offer payment plans through our financing partners:

• HFD: Check pre-qualification with no impact on your credit score — hfd.uno.dental.
• CareCredit: See our financing page for full details.

We also offer the in-house GleamBar membership, which covers preventive care and provides reduced rates on other services.

Insurance & billing

We accept all PPO plans on an out-of-pocket reimbursement basis. Our billing team will file the claim with your carrier on your behalf and help you maximize your benefits, while you choose your provider freely without network restrictions.

• Co-payments: All co-pays (patient portions) are due at the time of your appointment for services rendered. After your insurance processes the claim, you will receive a final statement. Any remaining or unpaid balances are the patient’s full responsibility.
• Payment authorization: By signing our intake forms, you authorize UNO DENTAL SAN FRANCISCO, Dr. Kandar, and associates to charge your card on file for any unpaid balances remaining after services are rendered if not paid in full prior to treatment.

Insurance disclaimers

• Treatment and insurance coverage estimates are based on information provided by your insurance carrier and are not a guarantee of payment.
• Insurance may pay less than estimated.
• UNO DENTAL SAN FRANCISCO will cooperate with your dental benefit plan and will not engage in disputes over claims.

Deposits, refabrication & estimates

• Deposits for significant cases: A deposit may be required to reserve time with the doctor and to contract our laboratory. The remaining balance must be paid in full at the first appointment and before treatment begins.
• Refabrication policy: Any refabrication (“redo”) of a rendered dental service will incur half the standard fee for the same service. Additional treatments required for success (e.g., crown lengthening, root canal, surgery) will be billed separately and are not included.
• Treatment estimates: Fee estimates are valid same day from the date presented.

Predictability & aesthetic outcomes

• You acknowledge that no medical or dental procedure is entirely predictable.
• Additional treatments beyond the initial plan may be required due to unforeseen circumstances; these may incur additional costs.
• Aesthetic outcomes are subjective. If you request a redo of a procedure for aesthetic reasons, you will be responsible for fees incurred (doctor’s time, lab costs, and overhead). The decision to redo for aesthetic reasons is at the doctor’s discretion, considering your oral health.

Professional fees, contact & media release

• Professional fees: You agree to pay for the professional services rendered to you or at your request. Should a suit be instituted for unpaid balances, you are responsible for costs and reasonable attorney fees.
• Contact permission: You authorize UNO DENTAL SAN FRANCISCO or our assignees to contact you at home or work to discuss matters related to this agreement.
• Media release: You grant UNO DENTAL SAN FRANCISCO permission to use your photographs for educational purposes, articles, publications, social media, website, television, and public relations.

1.2 Cancellation & appointment deposit policy

We value and respect your scheduled time with our dental team. By confirming an appointment, you acknowledge and agree to the following:

• Cancellations or rescheduling without 72 business hours’ notice will result in a full-fee charge for the scheduled services. We will assist you in rescheduling the same treatment at the earliest availability.
• All patients are required to provide a credit or debit card on file to secure appointments.
• Cards on file will not be charged until the day of your appointment, unless: (i) there is a last-minute cancellation of a confirmed treatment appointment; (ii) penalty fees for no-shows or cancellations are applied; or (iii) any outstanding balance remains unpaid.
• Treatment appointments require a deposit equal to 30% of the total treatment cost to reserve time in our schedule.
• Deposits will be forfeited if treatment appointments are canceled or rescheduled without 72 hours’ notice.

1.3 Consent for dental services

You authorize Dr. M. Kandar and associates to take all necessary diagnostic materials to establish a final diagnosis and treatment plan, including intra-oral photographs, digital radiographs, diagnostic models, and slides. You also consent to Dr. Kandar and partners performing or administering any treatments, medications, or anesthesia necessary for your care.

You permit UNO DENTAL SAN FRANCISCO to use diagnostic materials for educational purposes, including lectures, articles, and publications.

1.4 Refunds & warranties

We are committed to providing exceptional care. Warranties are valid only for patients who:

• Remain active in the practice for two years after treatment.
• Maintain doctor-recommended schedules for routine exams and hygiene visits.

Warranties are void if restorative work fails due to patient abuse, neglect, or misuse, including but not limited to:

• Biting into hard objects (e.g., metal, ice).
• Self-adjusting dental work.

If a night guard or occlusal guard is recommended to protect your restorative work, failure to have one fabricated or to wear it as directed voids the warranty. Choosing any treatment other than the primary recommended treatment also voids the warranty.

Refund policy

• Refunds are not issued for completed treatments.
• No refunds will be provided for rendered services under any circumstances.

1.5 Card on file authorization

Debit and credit cards are required for all appointments because our checkout and billing process is entirely remote — you will not be checking out at the end of your visit. We will not charge your payment method on file without your prior consent. You will first receive an itemized statement of the cost.

• We will bill your insurance for services rendered. Unless you receive a bill directly from us, you do not owe anything — please disregard any Explanation of Benefits (EOB) you receive from your insurance provider until our billing team has processed adjustments and write-offs in accordance with the provider’s participation agreement.
• A debit card (surcharge-free) is required for check-out and account balances. If you prefer not to provide a debit card, you may provide a credit card instead.
• An additional credit card (subject to a 3% surcharge fee) may be requested as a backup payment method.
• Card-on-file authorization is valid for one (1) year from the date signed.

A holder of this medical debt contract is prohibited by Section 1785.27 of the California Civil Code from furnishing any information related to this debt to a consumer credit reporting agency. In addition to any other penalties allowed by law, if a person knowingly violates that section by furnishing information regarding this debt to a consumer credit reporting agency, the debt shall be void and unenforceable.

13.1 Purpose of SMS text messages

We use Text Messages to streamline communication and enhance your experience. These messages may be used to:

• Communicate essential time-sensitive information related to your dental appointments.
• Share updates, special offers, or events related to UNO DENTAL services.

13.2 TCR (The Campaign Registry) compliance

In accordance with TCR requirements and RingCentral’s SMS guidelines, UNO DENTAL will:

• Obtain explicit consent before sending any marketing or informational Text Messages.
• Clearly identify ourselves as UNO DENTAL within each Text Message.
• Provide you with an easy and clear way to opt out of receiving further Text Messages at any time.

13.3 Frequency & rates

Up to 2 SMS messages per week regarding appointments or account status. Standard message and data rates apply per your carrier; fees vary domestically and internationally.

13.4 Opt-in methods

• Verbally during a phone conversation.
• Online form submission.
• Paper form completion.

13.5 Opt-out / unsubscribe

You may opt out of receiving Text Messages at any time. Reply STOP to any Text Message you receive. You may also reach out to us directly to revoke consent by calling 415-397-2804 or emailing hello@uno.dental. Reply HELP for assistance.

Standard messaging disclosures: Message and data rates apply. Phone numbers obtained through SMS consent are not shared with third parties or affiliates for marketing purposes.

13.6 Privacy & security

UNO DENTAL values your privacy. While we strive to protect your personal information, please note that Text Messages are not encrypted. There is a risk that unencrypted messages could be intercepted or accessed by unauthorized parties. By consenting, you acknowledge and accept that risk.

13.7 Your responsibilities

By consenting to receive Text Messages, you confirm that:

• You are the authorized user of the mobile phone number you have provided, or you have been granted permission to use it by the authorized user.
• You will notify UNO DENTAL if you change your mobile number or if you no longer wish to receive Text Messages.
• You agree to remain responsible for all fees charged by your mobile carrier in connection with the receipt of Text Messages.
