Thank you for selecting our office as your dental care provider. We are dedicated to delivering the highest quality dental care to support your long-term oral health. Payment is due at the time of service unless prior financial arrangements have been made.
We accept cash, personal checks, debit cards, HSA/FSA, VISA, American Express, MasterCard, and Discover. A 3% processing fee applies to credit card transactions; debit card payments are surcharge-free.

We accept all PPO plans on an out-of-pocket reimbursement basis. Our billing team will file the claim with your carrier on your behalf and help you maximize your benefits, while you choose your provider freely without network restrictions.
We value and respect your scheduled time with our dental team. By confirming an appointment, you acknowledge and agree to the following:
You authorize Dr. M. Kandar and associates to take all necessary diagnostic materials to establish a final diagnosis and treatment plan, including intra-oral photographs, digital radiographs, diagnostic models, and slides. You also consent to Dr. Kandar and partners performing or administering any treatments, medications, or anesthesia necessary for your care.
You permit UNO DENTAL SAN FRANCISCO to use diagnostic materials for educational purposes, including lectures, articles, and publications.

Warranties are void if restorative work fails due to patient abuse, neglect, or misuse, including but not limited to:
If a night guard or occlusal guard is recommended to protect your restorative work, failure to have one fabricated or to wear it as directed voids the warranty. Choosing any treatment other than the primary recommended treatment also voids the warranty.
Debit and credit cards are required for all appointments because our checkout and billing process is entirely remote — you will not be checking out at the end of your visit. We will not charge your payment method on file without your prior consent. You will first receive an itemized statement of the cost.
A holder of this medical debt contract is prohibited by Section 1785.27 of the California Civil Code from furnishing any information related to this debt to a consumer credit reporting agency. In addition to any other penalties allowed by law, if a person knowingly violates that section by furnishing information regarding this debt to a consumer credit reporting agency, the debt shall be void and unenforceable.
By providing your signature on our intake forms, you consent to the use and disclosure of your protected health information as outlined in our Notice of Privacy Practices.

True and accurate information. You agree to provide true, accurate, current, and complete information when prompted, and to maintain and update your account information. If we have reasonable grounds to believe your information is untrue or that you intend to violate these Terms, we may suspend or terminate your access in our sole discretion.
Access credentials. You are solely responsible for authorizing, monitoring, and controlling access to your access credentials and for any damages resulting from your failure to maintain confidentiality. Notify us immediately if you become aware of unauthorized use.
